Making Your First Custom Survey

  1. Click on the green New Survey button either at the top of the page or the gray New Survey button in the middle of the page.
  2. A pop up will open where you will need to enter a survey title in the first input field (this is required).
  3. Choose to have email addresses as Optional, Required or None in the Email drop down.
  4. If you’d like, enter a welcome message in the Welcome Message input field that will display at the beginning of your survey for your survey participants.
  5. Click Save.
  6. On the next page click the New Question button.
  7. This page is the question building page. On the left are all the options for your question. In the center of the page is the preview of your question.
  8. Type the question in the Question Title input field.
  9. Select the Required checkbox if you would like the question to be required.
  10. Select the question type from the Question Type drop down: Multiple Choice, Essay, Rating, Scale, Form, Scoring, Range, Date/Time or Image Choice.
  11. Based on the question type you choose, the options below that drop down will change. Enter any requirements, or answers you need for your question type.
  12. If you would like a subtitle to your question, click the Advanced Options arrow and enter the subtitle in the Subtitle input field.
  13. Click Save Question above the Question Preview to save your question to your survey.
